PCA FOR PATIENTS

In fact, the PCA team of physicians, nurses and social workers will make time to see you wherever and whenever it is convenient for you and your family, regardless if it’s an urgent need or a routine visit. What PCA Can Do for You

If you’re transitioning out of the hospital, the PCA team will visit you in the hospital to help arrange a smooth, comfortable transition home. Within 24 to 48 hours of discharge someone from our team will visit you, check your medications and arrange additional support resources that you may need. From there PCA will provide follow up visits as determined by the PCA team and your customized plan of care.

If you’re not transitioning from a hospital, but rather dealing with a serious or long term illness at home, our PCA team is available to assist. Please contact a team member and we will start the process by sending a clinical representative to your home when it’s convenient for you.

The PCA mission is simple: Be there to provide preferential medical support to patients that best manages their symptoms and facilitates a higher level of education related to their illness. With these expectations the PCA team has the following objectives:

  • Prevent the need for unnecessary hospitalizations or re-hospitalizations
  • Manage pain or other symptoms of a serious or long-term illness
  • Coordinate treatment with other care providers
  • Refer helpful community resources or specialty services
  • Verify that medications are taken as directed, and discuss possible side effects
  • Provide education and support to family or caregivers
  • Advise patients and family on advance directive decisions
  • Simplify a discharge from the hospital

We recognize that exceeding these objectives means addressing a patient’s concerns whenever needs arise. Because we take your health as seriously as you do, a member of the PCA team is available 24 hours a day, seven days a week.
